Any foreign body around implanted tissue (including breast implants) will form a capsule. In some cases, this capsule will contract. Contraction can deform the shape of the implant and the breast. Additionally, the capsule can become very hard and cause pain. This complication is known as "capsular contracture," and its occurrence is related to the surface of the implant. The first breast implants were manufactured in the 1960s with a smooth surface. Since the mid-1970s, implants covered with micro polyurethane have been in use. Textured surface implants were introduced in the late 1980s, and there are now three different surface materials. Although breast augmentation surgical techniques have improved, the capsular contracture rate of smooth implants is 30% compared to the very low capsular contracture rate of Microthane® micro polyurethane surface implants, which is only 0-3%! Warnings and Precautions
This product uses double sterile packaging. In the operating room, as long as the sterile inner packaging is not damaged or opened, the sterility of the product can be ensured. A backup implant should be available during the implantation surgery. When the sterile packaging is damaged, its sterility cannot be guaranteed. Do not use damaged breast implants. Avoid pollution
The surface of the implant must not be contaminated by talcum powder, dust, or skin oils. Due to static electricity, cotton fibers, talcum powder, or other surface contaminants may adhere to the implant, causing an increased foreign body reaction.
Special care must be taken to avoid contamination. Therefore, medical personnel involved in the operation should clean their sterile gloves with sterile saline solution before touching the implant.
Avoid damage during the surgical process
During the implantation surgery, care should be taken to avoid excessive force and to minimize manipulation of the implant.
When using surgical instruments near breast implants, attention should be paid to scalpels, suturing needles, and anatomical instruments. Silicone-filled breast implants may be subject to accidental mechanical trauma during the implantation or removal process. Scanning electron microscopy has observed shell ruptures on the implant shell, which may be caused by damage from instruments such as scalpels, suturing needles, injection needles, hemostatic clamps, retractors, needle holders, and forceps.
Please do not touch the implant with burning equipment.
Excessive manipulation may affect the mechanical properties of the implant, and this impact may not be detectable by the surgeon at the time of implantation.

It is essential to ensure that the organization has appropriate coverage, and if necessary, priority should be given to placing breast implants under the muscle.
It is advisable to avoid the direct combination of implants and medications.
Reuse and re-sterilization
Breast implants are for single use only and must not be reused or re-sterilized. The reasons are as follows:
After the implant is removed, no form of sterilization procedure on the surface characteristics of the implant is allowed or recommended.
Breast implants are very sensitive to low pressure/vacuum and may not withstand the re-sterilization procedures of medical devices.
According to international standards for the mechanical safety limits of new breast implants, it cannot be guaranteed that these limits will be met after the implants are removed, as stress is applied during the implantation and removal processes. Therefore, it cannot be guaranteed that reused products will still meet safety requirements.
Change the implant
Breast implants should never be altered before or during the implantation process. Each product should be checked for damage before implantation. Any damaged or altered products must not be used for implantation! A backup implant should be available during the implantation surgery.
Do not alter the implant, nor attempt to repair or implant a damaged implant.
Possible complications
Possible complications related to breast implants, in addition to the usual complications associated with invasive procedures, may also occur due to individual health conditions, medications, surgical techniques, and postoperative care. The following lists the intraoperative and postoperative risks associated with implants:
- Rash: The rash is characterized by redness of the skin, and its frequency depends on the different surface structures. The redness of the skin should not be confused with infection, as it usually does not accompany systemic infection symptoms and is associated with itching. The rash typically appears 7 to 10 days after implantation and can last for 2 to 3 weeks. If treatment is absolutely necessary, it is recommended to use steroids based on the literature.
- Infection: Infections associated with breast implants are very rare. Any unexplained infection occurring after implantation should be treated immediately. Antibiotics, drainage, or removal of the implant may be necessary. Bacterial cultures of wound secretions should be performed to determine the cause of the infection. Typical bacteria include: Staphylococcus aureus, Staphylococcus epidermidis, or Streptococcus. Not all infections can be treated while the breast implant remains in the body. There have been individual cases of toxic shock syndrome associated with breast implants.
- Inflammation or irritation: These are the body's responses to infection or injury, manifested as redness, swelling, or pain.
- Breast implant displacement, whether or not accompanied by persistent pain: this may be caused by improperly sized or incorrectly positioned implants. Possible causes include oversized implants, capsular contracture, and irritation caused by excessive movement of the implants. After breast surgery, pain may occur in the surgical area as well as in the chest muscle and shoulder area.
- Skin necrosis, shedding, exposure, and cracking: these complications may be caused by excessive tension on the skin from the implant (for example, the size of the implant is not compatible with the implant pocket) or interruption or reduction of blood flow to the skin (for example, trauma to the skin flap during surgery). Steroid treatment may also cause necrosis. Smokers are high-risk patients because blood circulation may be impaired. Corrective surgery may be necessary.
- Delayed wound healing: The wound healing period varies depending on the type of surgery and the location of the incision, and the healing time also differs among patients. In certain patients, especially smokers, significant delayed wound healing may occur. In this case, the risk of infection, exposure, and/or necrosis increases.
- Hematoma: A hematoma may be a precursor to infection and may increase the risk of capsular fibrosis. The occurrence of hematomas should be strictly avoided, and if a hematoma appears, it should be treated immediately. If not handled in a timely manner, significant postoperative edema or hematoma may lead to exposure or displacement of the implant. Non-absorbable hematomas may require immediate removal of the implant. Rigorous hemostasis during surgery is the best preventive measure.
- Wrinkles: Palpable implant edges or deformation may be due to differences between the implant size and the tissue pocket or insufficient tissue coverage.
- Capsular contracture or fibrosis: The fibrous tissue encapsulating the implant is a normal host response to the implant. However, this capsule may contract and harden, causing changes in the shape and position of the implant due to capsular contracture. If clinically significant (Baker III, IV) capsular contracture occurs, it may lead to aesthetic damage and pain, necessitating the removal of the implant. Closed capsulotomy is not recommended, as it may damage the breast implant. If such a procedure cannot be avoided, inspection procedures should be implemented during the surgery to prevent damage to the implant.
- Calcification: Benign calcification may occur around the implant.
- Implant rupture: Implant rupture or leakage is an absolute indication for immediate removal. The most common causes are damage to the implant during surgery (e.g., too short incisions, surgical instruments), post-surgery (e.g., punctures, biopsies, closed capsule incisions, etc.), trauma, or material fatigue. After a silicone-filled implant ruptures, the viscosity of the silicone can prevent displacement. However, there is no guarantee that the silicone will remain intact.
- Cancer risk: Epidemiological studies regarding implants show that the risk of breast cancer in women with implants is not higher than that of the control group.
- Desensitization or hypersensitivity: these may occur temporarily or permanently and may affect the breast completely or partially. Especially in cases where breast implants are inserted through an areola incision, sensation may change.
- Autoimmune and connective tissue diseases: Since the 1980s, the possible association between breast implants and these diseases has been discussed. So far, there is no evidence that breast implants cause these diseases.
- Changes in breastfeeding ability: Breast surgery may affect breastfeeding ability.
- Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL): Several studies have indicated a potential association between breast implants and the occurrence of ALCL. This means that women with breast implants may have a slightly increased risk of ALCL. Patients should be informed by the responsible surgeon about the low risk of ALCL and advised to consult their attending physician if symptoms such as seroma or swelling and pain in the implant area occur more than a year after surgery.
- Axillary lymphadenopathy
- Breast tissue/chest wall deformation: The skin covering the implant may thin due to the tension it exerts, or the pressure from the implant may deform the ribs. This may lead to the need for revision surgery.
- Impact on infants: Currently, there is no evidence to prove that breast implants pose a health risk to infants born to or breastfed by mothers with implants.
- Unsatisfactory cosmetic results: For both patients and doctors, dissatisfaction with shape or size can be minimized through careful surgical planning. Revision surgery can provide assistance, but it also comes with new risks.
- Asymmetry: The breasts present varying degrees of asymmetry in size, shape, and/or position. Pre-operative asymmetry may not be fully compensated for with implants and surgery.
- Sagging: The relaxation of breast tissue may be caused by gravity, pregnancy, weight loss, and aging. In one
